Question about masquerade ball dress code, and parental permission forms

the parental permission form my parents signed it when we got my badge and dated it 5/8/08 will it be accepted?

and are these shoes acceptable for the masquerade ball? I'm going for a dressy tifa form and I just wanna make sure these are accepted.

I believe they are. I wore flip flops last year because my heels were killing me after awhile. the only things you need to get in is either a mask or cosplay.

The main requirements for the Masquerade Ball is formal wear (nice dress or slacks/shirt/tie), or cosplay.. and you must be wear a mask.  Comfortable shoes are always great :) (your shoes are fine).
