Now my question is about Party B. Do they have to sign the document as well?
Don't know, will replace this with answer soon.
Party B does not need to sign it. If Party B doesn't act as a proxy for Party A, then that's Party A's problem, not Party B's. The contract is between Party A and B, not between Party A and the Organization. If they have issues, they can deal with it between each other. I really don't care one way or the other, as long as said contract is legit. =P Much like if you let a friend borrow your car, and that friend got pulled over while driving it. As a "cop" all I'm interested in is whether or not you actually did loan the car to the person, or whether the car is "stolen".

Do they need to be a staff member?

No, the person acting as a proxy does not need to be staff (But, like staff, that person will need photo ID to claim the ballot). For the duration of the elections, they are basically being given the "staff hat" of the person who is entrusting it to them. That Party B can do whatever they want with it... vote, abstain, whatever. Again, that's between A and B.

And, I don't necessarily need to get the affidavit before the meeting. Getting it before would be nice, but as long as I have it the day of (before the actual voting process begins), then we're good.

Thanks! And good questions. I seriously hope that very few people will be doing this... giving a vote to someone else, and not being present to actually hear and participate in the process removes most of the actual value of the voting rights.

I agree with that last comment, other than in the case of someone who has been actively engaging with the candidates through the threads on the forums but who simply has an insurmountable obstacle to physical attendance at the meeting, such as being unable to get the day off work (or studying abroad in Japan!).

If you cannot reach the proxy voter by phone to confirm their vote does it still count?

If I cannot reach the person who is having a proxy vote in their stead, and the document was not notarized, then that proxy will NOT receive a ballot. So, it's not even that their vote won't count, they won't get a vote. I am not calling to confirm how the proxy voted... I won't know that information (no one but the proxy will know how the proxy voted. Hence the risk in appointing a proxy if you do not completely trust them to act on your behalf). I am only calling to make sure the affidavit is legit before they receive the ballots.

Sorry. =/

As a reminder... most people who have bank accounts, or credit union accounts, should be able to go into their bank/CU and have something notarized for free or a small fee. They will need 2 witnesses, but generally, other staff at the bank can serve in this function.

Thank you, as my husband has to work (he's k-con staff) and I'm really the only one he trusts.  Is it too late to have me act as his proxy?

As long as you bring the statement with you tomorrow and Staze gets the notarized document saying you can act as his proxy tomorrow before elections starts or, if it's not notarized, Staze can contact him before the meeting starts, you should be ok.

Since some people like structure and hate typing....

Then again, if you don't mind a little typing, print out this fillable form.
http://pockyclub.elite-otaku.net/proxycontract_fillable.pdf

This is a fillable pdf where you can type everything in, print it out and sign it.
I went to the trouble of making this because I figured it would be good to have a nice little form for use in any election or vote that came up in future years.

Tom, though I can't open attachments at work so I haven't seen the actual text (and I'm not using or asking anyone else to be a proxy at least as of this point), I appreciate that you thought to create a shortcut for anyone who needs one. :)

When I first read it, I thought it said pocky contract instead of proxy
